What causes acne scars?

Acne scarring can develop when inflammation associated with acne affects the deeper layers of the skin. As the skin heals, the body produces collagen to repair the affected tissue. The amount of collagen produced during this process can influence the type of scar that forms. Some people may develop too little collagen, resulting in depressed (atrophic) scars, while others may produce excess collagen, leading to raised (hypertrophic or keloid) scars.

 

Not everyone who experiences acne will develop scarring, and the severity of scarring varies from person to person.

What types of acne scars are there?

Acne scars are generally classified into two main categories: atrophic scars and hypertrophic (keloid) scars.

Atrophic scars

There are three types of Atrophic scars:

  • Atrophic scars develop when insufficient collagen is produced during the healing process. The three most common types are:

    • Boxcar scars – Broad, well-defined depressions with sharp edges, commonly found on the cheeks and jawline.
    • Ice-pick scars – Narrow, deep scars that extend into the skin and are commonly seen on the cheeks.
    • Rolling scars – Broad depressions with gently sloping edges that give the skin an uneven or wavy appearance.

Hypertrophic (keloid scars)

Hypertrophic scars develop when excess collagen is produced during healing, resulting in raised areas of scar tissue. Keloid scars are a specific type of raised scar that may extend beyond the original area of skin injury in some individuals.
